Common Bile Duct (CBD) & Intrahepatic Biliary System Evaluation

A-Normal CBD Diameter

Adults: ≤6 mm (The normal diameter of the common bile duct (CBD) on an ultrasound is between 1.8 mm and 5.9 mm).

Elderly (over 60 years): Can be up to 7–8 mm

Post-cholecystectomy: Can be up to 10 mm (due to compensatory dilation)

After meals: May decrease slightly

 

Dilatation (>7 mm) suggests biliary obstruction (stone, tumor, or stricture).

Double Duct Sign: Dilated CBD & Pancreatic Duct → Suggests obstructive pathology (e.g., Pancreatic tumor).

 

B-Biliary System Evaluation

·         Intrahepatic Bile Ducts: Normally not dilated; dilation suggests cholestasis, obstruction.
